Living Well with Chronic Conditions

Living Well with Chronic Conditions is a chronic disease self-management program that was developed at Stanford University and has been adopted by many community partners and hospital systems throughout the country. It is a free six-week course that teaches practical skills for living a happy and healthy life with an ongoing health condition. This interactive, fun workshop meets for two and a half hours each week. The workshop is led by a pair of specially trained volunteer leaders.


The Living Well “Toolbox”

Throughout the six sessions, participants will acquire skills such as:
  • Health literacy: learning how to work with your health care provider
  • Healthy eating and physical activity
  • Relaxation techniques
  • Communication skills and problem solving
  • Decision making
  • Better breathing and pain management

Volunteer Opportunities

Living Well with Chronic Conditions is always looking for volunteers to lead workshops.
