Samaritan Evergreen Hospice
— which serves Albany, Corvallis and Lebanon — introduces a new program called Samaritan Transitions.
The volunteer-based Transitions program serves members of the community who are experiencing a lifelimiting illness with a prognosis of 12 months or less.
Transitions is non-medical, and offered in people’s homes. The objective of the program is to connect individuals with a volunteer who can help them access community resources.
With help, individuals may be able to maintain a degree of independence during a time of serious illness. Samaritan Transitions is currently recruiting volunteers to help with a variety of tasks — from running errands, food shopping, and pet care to offering companionship and emotional support, transportation when appropriate, and respite for a caregiver.
Volunteers will be trained to understand the program’s structure and gain knowledge of community resources. The level of commitment is 2 to 4 hours per week. Transitions volunteers must be 18 years of age or older.
