Join Rick Wopat, MD, of Samaritan Medical Home and get your questions answered.

As the state of Oregon implements health care reform, health care providers in our region have been hard at work transforming many primary care clinics into patient-centered medical homes. Patient-centered medical homes coordinate your physical and mental health care to help get you the services you need, when you need them, and help you play an active role in your health.

Find out what patient-centered medical homes mean for you and your health care at this free seminar on Tuesday, Sept. 17, from 6 to 7:30 p.m., in conference rooms B and C, at Samaritan Lebanon Community Hospital, 525 N Santiam Hwy.

Healthy refreshments will be provided. Space is limited, and registration is required. Visit samhealth.org/BeHealthy, or call 541-768-4887, or toll free at 1-855-873-0647 to register.
