Learn about end-of-life health care options and Advance Directives at a KeyConversations workshop at Good Samaritan Regional Medical Center.
The workshop will help you understand your choices for medical care and life-sustaining treatments, legally document your decisions and appoint someone to make health care decisions for you when you are unable to speak for yourself.
Workshop participants will receive a free KeyConversations planning guide, designed to help you talk with your loved ones about your end-of-life decisions while completing your Advance Directive. Workshops will be held in Good Sam conference rooms on the following dates:
Thursday, Oct. 15, 4 to 5 p.m. and 7 to 8 p.m.
Wednesday, Nov. 4, 7 to 8 p.m.
