Making your wishes known can be one of the most meaningful gifts you can give to your friends and family. Join Dr. Takiko May and the Rev. Wes Sedlacek for more information and insight in determining your "quality of life statement" and your wishes for care when potentially facing your mortality.
Although there are often many medical interventions that can be made, not all of them are appropriate for every person. Exploring your personal values can help you better determine what you would want for yourself if you are ever faced with such life-prolonging decisions.
During this presentation you will be given general advanced medical planning information as well as exposure to the Advance Directive and Physician Orders for Life Sustaining Treatment (POLST) documents. These documents are designed to make sure your wishes are known by your health care providers and loved ones in the event that you lose the capacity to voice your wishes due to illness.
