It's easy to enroll in the Samaritan Advantage plan of your choice:
ENROLL OVER THE PHONE. Once you have decided which plan you would like to enroll in, you can complete your enrollment over the phone. It can't be easier! Just call (541) 768-4550 or 1-800-832-4580 (TTY 1-800-735-2900), Monday - Friday, 8 a.m. to 5 p.m., and press "1" for phone enrollment when prompted. One of our enrollment experts will assist you in completing your enrollment.
ENROLL IN-PERSON. Stop by our offices in Corvallis at 815 NW Ninth Street, Monday – Friday, 8:30 a.m to 5 p.m. One of our friendly staff can assist you, if needed, in completing your enrollment form.
ENROLL BY MAIL OR FAX. Simply download the enrollment form (right), complete the form and mail or fax the form to us.

The Medicare Annual Enrollment Period for 2012 benefits ends on December 7, 2011. To have Medicare Part D prescription coverage or other Medicare Advantage benefits on January 1, 2012, you must have enrolled in a plan by December 7, 2011.
EXCEPTIONS: You may apply for Samaritan Advantage coverage within 3 months of your 65th birthday, no matter when that birthday occurs. Additionally, some beneficiaries, including those that are disabled as defined by Medicare, are allowed to enroll in or switch plans at other times.
