2012 Enrollment requirements

To become enrolled in the Samaritan Choice Wellness Plan for 2012, you will have some additional requirements to meet. These requirements apply only to employees who wish to enroll in the Samaritan Choice Wellness Plan. They do not apply to your spouse or domestic partner and dependents. If you meet the enrollment requirements, your spouse or domestic partner and dependents all qualify for enrollment in the Wellness Plan for 2012.

If you choose to enroll in the Samaritan Choice Basic or High-Deductible plans, you do not have any additional requirements to meet, however, your out-of-pocket costs will be higher. See plan documents.  See plan comparison.

To complete enrollment in the Samaritan Choice Wellness Plan for 2012, you will need to:

  • Complete a biometric screening test, see below
  • Complete an online health assessment questionnaire, see below
  • Sign an online acknowledgement form, see below

Biometric screening for Wellness Plan 

The free, confidential biometric screening will provide you with vital numbers (such as cholesterol, blood glucose, blood pressure, and more) that you can compare to recommended healthy guidelines to help determine your risk for disease and chronic health conditions. Contact your local HR department to arrange for your biometric screening.

Have recent lab work? It may qualify as a substitute.
If you have completed a biometric screening after July 1, 2011, either through the Samaritan Employee Wellness HRA’s or your personal physician, you can submit the lab results to our screening vendor, Vivacity (www.vivacity.net), instead of participating in the biometric screening. Lab results must include a minimum of Height, Weight, Blood Pressure, Glucose, Total Cholesterol, HDL, and Coronary Risk Ratio. Use this form to submit your results. 


Online health assessment for Wellness Plan

The online health assessment questionnaire is a secure online tool that you will use to provide information about your health. After you complete the health assessment, you will receive a summary report that identifies areas you are doing well in, as well as areas of potential health risks and what you can do to reduce those risks. You must complete this online health assessment questionnaire even though you may have completed a similar questionnaire as part of your Samaritan Employee Wellness HRA.

Online acknowledgement form

The online acknowledgement form reviews the enrollment requirements of the Wellness Plan. You will need to agree to the requirements before you can complete your online health assessment questionnaire. The acknowledgement form will be part of your online enrollment process.
