More support to improve and maintain your health

We have developed support programs to compliment the medical advice of your health care provider. All services provided by these programs are covered 100% by the Samaritan Choice Wellness Plan. Eligibility criteria apply. If identified as a member who would benefit from these services, you may be required to participate in these programs.

Health coaching
These one-on-one services will help you:
  • Identify what is motivating you to make lifestyle changes
  • Set specific, measurable, attainable, relevant and time-limited goals
  • Identify barriers and create steps to overcome the barriers
  • Build skills to find reliable health information and resources specific to your needs.

Care coordination
Care coordinators facilitate and advocate for options and services to meet an individual’s health needs.

Primary Care Home
The PCH is responsible for meeting the large majority of each patient’s physical and mental health care needs, including prevention and wellness, acute care, and chronic care. They coordinate care across all elements of the broader health care system, including specialty care, hospitals, home health care, and community services.

What hasn't changed for 2013

The quality of your benefits.
You will continue to receive comprehensive coverage for your health care needs and the personal service you have come to expect from Samaritan Choice Plans.

Your provider network.
No matter what plan you choose, you will have the same extensive network of providers that are located in your hometown and beyond. Continuing to use these network providers will help you manage your out-of-pocket costs.

The protection of your privacy
All information from your health plan, health assessment and biometric screenings are confidential and protected by The Health Insurance Portability & Accountability Act of 1996 (HIPAA) Privacy Rules.

Specific personal health information (PHI) received and gathered, as a result of your biometric screening and/or health assessment, will NOT be shared with your employer for any reason. Your employer will receive an aggregate summary only. There will be no personally identifying information shared with Samaritan Health Services.

No cost to you for preventive care and many other services
No matter what Samaritan Choice Plan you choose, you will continue to have your preventive care covered 100% when received from network providers. These preventive services are: well baby care visits, routine physicals, routine gynecological exams, comprehensive women's exams, immunizations, and colonoscopies.

Other network services that will continue to be covered at 100% are: skilled nursing facility care, diagnostic and therapeutic radiology and labs (excluding CT Scans, PET Scans, and MRI’s), and hospice care.

NEW physical therapy benefit

Direct access to physical therapy services from a licensed physical therapist for physical therapy are covered. This service does not require a physician referral; members may self refer.

SamFit conditioning and training option
When a Samaritan Physical Therapist refers a member for SamFit training and conditioning, in accordance to the criteria below, the member may have up to three months of a SamFit gym membership paid at one time.
  • Member must benefit from physical conditioning to support the member’s recovery of the condition treated, while reducing the risk of a reoccurrence.
  • The member must require additional assistance in performing specific rehabilitative exercises, so that further injuries can be avoided.
Additional services may be reimbursed through additional review and referral of a Samaritan Physical Therapist supporting that the member meets the criteria above. Services will only be reimbursed if the member is eligible under the plan at the time services are rendered and only when provided through the SamFit locations and appropriate staff where required.
