Enrollment requirements for the 2016 Samaritan Choice Wellness Plan

If you are not currently a member of a Samaritan Choice plan, please follow the instructions below and view your Wellness Plan agreement.

Current members of Samaritan Choice plans should access the member portal at MyHealthPlan.samhealth.org to complete the enrollment requirements.

New incentives for 2016

This year, the Wellness Plan credit has been replaced with new incentives that reward employees and spouses or domestic partners for completing a biometric screening and personal health assessment and enrolling in the 2016 Samaritan Choice Wellness Plan. The 2016 incentive is a Samaritan Choice Wellness PLUS $150 benefit for each!

The enrollment requirements to qualify for this new incentive are: Complete a biometric screening and personal health assessment between Aug. 10 and Sept. 4.

Personal health assessment

Employees and their spouses or domestic partners can take the personal health assessment between Aug. 10 and Sept. 4. A link will be made available here beginning Aug. 10.

IMPORTANT: Employees are required to complete a biometric screening and personal health assessment to join the Wellness Plan; for spouses or domestic partners, completion is required to receive the incentive.
