Sign-up by Nov. 6 for your biometric screening for Samaritan Choice Wellness Plan through an online scheduler  - see dates and locations

  • Oct. 8 - Nov. 6 Schedule your biometric screening if you plan to enroll in the Samaritan Choice Wellness Plan. Read your STEP-BY-STEP INSTRUCTIONS to schedule your biometric screening. Note: Appointments must be made 48 hrs. in advance of an event.

You must complete your Health Risk Assessment online prior to enrolling in the Samaritan Choice Wellness Plan.

Attend your biometric screening appointment if you plan to enroll in the Samaritan Choice Wellness Plan. See dates and locations.


Open Enrollment
IF YOU ARE SATISFIED with your current Samaritan Choice health plan, you DO NOT have to complete an online enrollment form during Open Enrollment, Nov. 5 - 16, 2012.

You MUSTcomplete online enrollment IF you want to:

  • Make any changes to your current plan or choose another plan — these options are only available during the Open Enrollment period, Nov. 5 - 16, 2012.
    - OR -
  • Enroll in a Samaritan Choice health plan for the first time.
