Enrollment requirements for Samaritan Choice Wellness Plan

To become enrolled in the Samaritan Choice Wellness Plan, you will have some additional requirements to meet. These requirements apply only to employees who wish to enroll in the Samaritan Choice Wellness Plan. They do not apply to your spouse or domestic partner and dependents. If you meet the enrollment requirements, your spouse or domestic partner and dependents all qualify for enrollment in the Wellness Plan.

If you choose to enroll in the Samaritan Choice Basic or High-Deductible plans, you do not have any additional requirements to meet, however, your out-of-pocket costs will be higher. See plan comparison.  See plan documents. 

To complete enrollment in the Samaritan Choice Wellness Plan, you will need to:

  1. Complete a free biometric screening test
  2. Complete an online health self-assessment
  3. Sign an online acknowledgement form as part of your enrollment for employee benefits - this applies to first-time members only.

Free biometric screening
The free, confidential biometric screening will provide you with vital numbers (such as cholesterol, blood glucose, blood pressure, and more) that you can compare to recommended healthy guidelines to help determine your risk for disease and chronic health conditions. 

Individual results from your biometric screening remain confidential and are not shared with Samaritan Health Services. It is against the law for your employer to access your personal health information. Samaritan Health Services, and you as an employee, will only see the combined results in a Health Status Report. You can view the current report on SHS Insider in the Everyday Choices website, found under “Wellness” in the main navigation.


Online health self-assessment
The health self-assessment is a secure online tool that you will use to provide information about your health. This tool allows you to self-assess your health and immediately receive a report that identifies areas of potential health risks and what you can do to reduce those risks. If you completed a self-assessment last year as part of your Samaritan Choice Wellness Plan enrollment, those results are still available to you for comparison.

If you intend to enroll in the Wellness Plan for 2013, you will need to complete the online health self-assessment.    
Online acknowledgement form - applies to first-time members only
The online acknowledgement form reviews the enrollment requirements of the Wellness Plan. You will need to agree to the requirements before you can complete your online health assessment questionnaire. The acknowledgement form will be part of your online enrollment process.
