How to enroll in your health plan for 2012

You have three Samaritan Choice Plans from which to choose: 
  1. Wellness

  2. Basic

  3. High-Deductible

Please follow the steps below to enroll in a health plan for 2012.

Step 1.  Biometric screening

If you want to enroll in the Basic or High-Deductible Plan, skip this step and go to Step 2.

If you want to enroll in the Wellness Plan for extra wellness benefits and lower out-of-pocket costs, you will need to complete your required biometric screening. Contact your local HR department to arrange.

A biometric screening is not required if you send your lab results from a physician visit or from your Employee Wellness HRA to Vivacity. See below for instructions on how to submit your results. Click here to learn more about the biometric screening.

Step 2.  Enroll online

Go to http://www.electbenefits.com/samaritan to enroll.  Click here for detailed online enrollment instructions.

  • Your login is your 5-digit employee ID
  • Your pin number is MMYY of your date of birth.
If you have elected to enroll in either the Basic or High-Deductible Plan, you have completed your enrollment. 

If you have elected to enroll in the Wellness Plan, you must complete Step 3.

Step 3.  Online health assessment questionnaire

Go to www.Vivacity4me.com to complete your online health assessment questionnaire.  This is required if  you have elected to enroll in the Wellness Plan. .

Click on the "Vivacity" button to activate and log in to your account.

Click the "Activate Benefits" button under "First Time User" and follow the instructions. To complete your registration, you will need:

  • Your first name and last name as reported by you to HR
  • Date of birth (M/D/YYYY)
  • Gender
  • User ID which is SHS-xxxxx-00 (replace the x's with your employee ID number; be sure to insert the dashes)
  • If you are unable to login to the Vivacity website, please contact us for assistance.
You will be asked to provide information about yourself to set-up account access. This includes creating a login name and password that will allow you secure access to your health risk assessment and a personalized portal to valuable health programs and tools that you and your family can use at home. Be sure to save your Vivacity login and password information in a safe place for your future use.

Click on "Personal Health Assessment" link.
Follow the instructions provided and answer all questions within the health assessment questionnaire.  Answer the biometric questions to the best of your ability if you do not have your results.  The results from your biometric screening will be imported into your health assessment at a later date. How to submit your lab results from a physician visit or your Samaritan Employee Wellness HRA

Step 1 is not required if you send your lab results from a physician visit or from your Employee Wellness HRA to Vivacity. Use this form to submit your results. Lab results must have been completed on or after July 1, 2011, and include height, weight, blood pressure, glucose, total cholesterol and HDL measurements, and a coronary risk ratio.
