How to enroll in your health plan

You have three Samaritan Choice Plans from which to choose for 2013: 

1. Wellness

2. Basic

3. High-Deductible

If you are satisfied with your current Samaritan Choice health plan, you DO NOT have to complete an online enrollment form during Open Enrollment, Nov. 5 - 16, 2012.

You must complete online enrollment if you want to:

• Make any changes to your current plan or choose another plan — these options are only available during the Open Enrollment period, Nov. 5 - 16, 2012.
  - OR -
• Enroll in a Samaritan Choice health plan for the first time.
