Getting Started: Identification Cards

Your OMAP Medical Care Identification (ID)

When you become an Oregon Health Plan (OHP) client, the Office of Medical Assistance Programs (OMAP) will mail you a Medical Care Identification (ID) each month.  This identification is a mauve and white full sheet of paper printed on both sides.  It is important to carry this identification with you whenever you receive services covered by the Oregon Health Plan.

The Medical Care ID shows you important information about your benefit package. As an IHN member with the OHP Plus package you are fully covered under the Oregon Health Plan. As an IHN member with the OHP Standard package your services are limited, and you will have to pay copayments.

Your IHN Member Identification (ID) card

You will receive an IHN Member ID Card once you have chosen a PCP. You should present this card whenever you need medical services. It lists information about you, the name of your PCP and his/her phone number. It tells you what to do in case of an emergency. It also shows you what benefit package you are eligible for (OHP Plus or OHP Standard). If you lose your IHN Member ID Card, please call us and we will send you a new one. You will also receive a new one any time you change your PCP.
