The goal of the clinic is to create a comprehensive approach to improve health outcomes.
The medical team includes a physician, two nurse practitioners, a case manager to coordinate your care, a behavioral health counselor, an appointment coordinator and a health coach. Our goal is to improve the quality of care you already receive and to make sure that patients are seen the same day they call for an appointment.
