Enroll over the phone

Once you have decided which plan you would like to enroll in, you can complete your enrollment over the phone. It can't be easier! Just call 541-768-4550 or 1-800-832-4580 (TTY 1-800-735-2900), Monday – Friday, 8:30 a.m. to 5 p.m. One of our enrollment experts will assist you in completing your enrollment.

Enroll in person

Stop by our offices in Corvallis at 815 NW Ninth Street, Monday – Friday, 8:30 a.m. to 5 p.m. One of our friendly staff can assist you, if needed, in completing your enrollment form.

Enroll by mail or fax

Simply complete our enrollment form and mail or fax the form to us: Mail to Samaritan Health Plans HMO, Attn: Enrollment Dept., PO Box M, Corvallis, OR 97339. Fax to 541-768-9778, Attn: Enrollment Dept.

Other options

Medicare beneficiaries may also enroll in a Samaritan Advantage Health Plan HMO through the CMS Medicare Online Enrollment Center located at www.medicare.gov.

Samaritan Advantage Health Plan HMO is a coordinated care plan with a Medicare Advantage Contract and a contract with the Oregon Medicaid program. Enrollment in Samaritan Advantage Health Plan HMO depends upon contract renewal. Individuals must have both Medicare Part A and Part B to enroll. Members may enroll in the Conventional, Premier, and Premier Plus plans only during specific times of the year. You must continue to pay your Medicare Part B premiums. The Part B premium is covered for full-dual members. You must receive all routine care from plan providers. Benefits, formulary, pharmacy network, provider network, premium, and/or co-payments/coinsurance may change on January 1 of each year.
