Medical Record Request

Being able to access your medical record is an important component in our partnership toward improving your health.

But before we can release your medical record, we are required to receive specific written information from you first. Simply follow the steps below, and we'll work with you to securely transfer your medical record to you, your doctor or health insurer as quickly as possible.

Requesting a copy of your medical record

If you are looking for your medical record as it relates to a past stay at one of our hospitals or any of our medical clinics throughout Linn, Benton and Lincoln counties, you will need to call our Medical Records Department at the hospital in the city where you or your legal dependent received care. Our medical records staff will provide you with the necessary medical record release paperwork. Then simply fill out the form provided and return it to the appropriate department.
