Discharge Planning
Both social workers and nurses help patients and their families coordinate any needed post-hospital care and services. These professionals also offer the following services to all patients and their families:
- Assistance with pre-hospitalization discharge planning
- Consultation regarding advance directives and directives to physicians
- Referrals, when indicated, to home health and other outreach services; level services; help with financial arrangements; mental health services; arrangements for medical equipment; and support groups
In addition, the social services staff members are available to help patients and families cope with emotional concerns such as:
- Separation from family and friends
- Long-term illness and rehabilitation
- The need for surgery
- Terminal illness
- Alcohol or other drug problems
- Abuse, rape, trauma, etc.
- Special needs of children
- Discharge or transfer to another facility
- Acute emotional stress
