Park Street Clinic in Lebanon has become the first Samaritan Health Services adult family medicine clinic to earn a 5-star rating from the Oregon Health Authority (OHA).
OHA auditors awarded the 5-star designation – which is the highest achievable - following a recent clinic survey as part of the state’s Patient Centered Primary Care Home (PCPCH) program. This program is available to primary care clinics that include behavioral health, case management and other support services who work with the patient alongside the primary care provider.
“PCPCH facilities are health care clinics that have been recognized by the Oregon Health Authority for their commitment to providing high quality, patient-centered care,” said Kathy Cowger, clinic operations director for clinics associated with Samaritan Lebanon Community Hospital. “At its heart, this model of care fosters strong relationships with patients and their families to better care for the whole person. Primary care homes reduce costs and improve care by catching problems early and focusing on prevention, wellness, and managing chronic conditions.”
Cowger and Molly Bell, clinic manager, said the Park Street Clinic team is honored to receive the 5-star designation. “This is a direct result of the clinic’s hard work and commitment to our patients and our community,” Cowger said. “I am proud to work with this team and congratulate them on such an amazing accomplishment.”
