Susan Tolle, MD from the Center for Ethics in Health Care at Oregon Health & Science University is presenting a workshop about Portable Orders for Life-Sustaining Treatment (POLST) from 11 a.m. to noon on May 9, at Samaritan Evergreen Hospice.
Caregivers, social workers, faith leaders, elder law attorneys, health professionals and hospice volunteers are encouraged to attend and learn more about supporting people as they become ill and frail. Dr. Tolle will teach vocabulary and skills needed to help individuals talk about what matters most with their health care professionals and others as they complete a POLST form.
Besides learning how to be an advocate for informed decision-making near the end of life, attendees will have the opportunity to sign up to receive the latest updates and changes to the Oregon POLST form.
