Nonprofit organizations in Benton, Lincoln and Linn counties interested in applying for Social Accountability funding from one of the five Samaritan Health Services hospitals are invited to submit a full proposal no later than 4 p.m. on Friday, Oct. 1.
A complete proposal must address one of the following social determinants of health:
- Economic stability.
- Education access and quality.
- Health care access and quality.
- Neighborhood and built environment.
- Social and community context.
Specifically, a project must identify how the project will address the particular determinant of health, identify the target population the project will address and must have measurable outputs.
Agencies that are planning to apply are highly encouraged to attend an online grant information meeting on Thursday, Aug. 26, at 10 a.m., using the Microsoft Teams platform.
