If you live, work, go to school or spend time in Benton, Lincoln or Linn counties, you are invited to share your thoughts about the health of your community. The deadline has been extended to Friday, June 17. Please participate in the survey by then to provide important input and help to make positive change in your community.
Results from the Regional Health Assessment will be used by the three county health departments, the Confederated Tribes of Siletz Indians, Samaritan Health Services and the InterCommunity Health Network CCO to develop plans to improve community health.
The RHA survey is conducted every three to five years to find out what affects people’s physical, behavioral and social well-being. Input is needed from a diverse set of community members for the most effective community health planning.
All responses are anonymous and any information you provide is useful.
