Diabetes & Nutrition Services

If you have been diagnosed with diabetes, pre-diabetes or are interested in learning more about healthy living, we can help you get on the right track.  

Our team or professionals includes nurses and certified diabetes educators who listen and provide guidance based on your wants and needs.

Our services

  • One-on-one counseling – diabetes and/or nutrition
  • Foot clinic with services including toenail trimming and debridement, for people diagnosed with diabetes
  • Metabolic testing

 *Provider referral required

Our classes

We also encourage you to participate in our classes, including:

  • Diabetes Self-Management*: Three-week class
  • Diabetes Prevention: Eight-week class, registration required
  • Diabetes Support Group: Meets monthly, registration encouraged
  • Complete Health Improvement Program (CHIP)*: Nine-week class, registration required
  • Plant-Based Kitchen: Monthly classes with topics related to the plant-based diet, registration required
