Samaritan Is Experiencing Verizon Phone Outages

Phone calls made using Verizon are currently not coming through at Samaritan facilities.  We are working with Verizon to resolve this issue as soon as possible. If you need to call, please use a phone with an alternate phone carrier or a land-line phone if possible. 

Welcome to Samaritan Lebanon Health Center

Our goal is to provide great health care for the whole family. At the Samaritan Lebanon Health Center, we provide a wide range of health care for patients of all ages, including:

Our goal is to provide the best possible care through all stages of your life. Your health care team may include doctors, nurse practitioners, physician assistants, behavioral health specialists, medical assistants and other clinic staff members working together to help you maintain the best possible health.

We encourage and offer preventive care including annual exams, health screenings, and counseling regarding health and lifestyle.

You are welcome to use your MyChart patient portal to schedule appointments. When logged in to the MyChart patient portal, just click Appointments & Visits at the top of your screen. Follow the prompts to schedule a visit with your primary care provider.

We are honored that you have trusted us with your child’s care. Our clinic offers complete care for children from birth through the teenage years.  

Your child’s health care team may include doctors, behavioral health specialists, medical assistants and other clinic staff members working together to help keep your child healthy.

We are dedicated to providing the highest quality health care and encourage preventive care, including well-child visits, vaccinations and sports physicals. We also will coordinate any care needed from other clinics or specialists, and we will arrange any necessary referrals.
