Reasons for Home Health

Below are some questions as you consider if home health services are right for your patient. Is he or she:

  • Living with chronic conditions, such as COPD, diabetes, congestive heart failure, kidney disease or high blood pressure
  • Recovering from surgery or a serious medical event, such as a heart attack or stroke
  • Needing help with wound care
  • Needing help understanding and managing their medication
  • On dietary requirements 
  • Needing treatment progress  to be monitored
  • Having mobility challenges or is afraid of falling

Determining Homebound Status

Medicare defines someone as “homebound” if a person is: 

  • Is unable to leave home due to an illness or injury
  • Endures “considerable and taxing effort” in order to leave home
  • Leaves home infrequently and for short periods of time to receive medical care that cannot be provided in the home

As you consider if your patient is eligible for home health care services, ask these questions: 

  • Does the patient safely leave home for non-medical purposes? 
  • Does the patient require assistance when leaving home? 
  • Does the patient endure considerable and taxing effort to leave home? 
  • Does the patient needs skilled services, such as registered nursing care, mental health nursing, physical therapy or speech therapy?
