Attend an class to ask questions about an Advanced Directive Form and learn how to complete yours.

The Advance Directive spells out a person’s wishes in case that person can’t communicate for some reason, whether due to an accident or serious illness. These classes will help make a confusing document more understandable.  

Hospital Chaplain Wes Sedlacek and Licensed Clinical Social Worker Heather Krimsley, will lead the classes, and help people to complete their own Advance Directive during each session.

Physician Order for Life Sustaining Treatment (POLST) forms, which is another document used to specify a person’s wishes for medical treatment, including CPR will also be covered n the class.   

Drop-ins are welcome but registration is requested, either by registering online or calling 541-812-4960.
