Samaritan Lebanon Community Hospital will offer monthly classes for anyone with questions about how to fill out an Advance Directive form.

The Advance Directive spells out a person’s wishes in case that person can’t communicate for some reason, whether due to an accident or serious illness. These classes will help make a confusing document more understandable.  

Hospital Chaplain Wes Sedlacek, who leads the classes, will help people to complete their own Advance Directive during each session.

Sedlacek will also discuss the Physician Order for Life Sustaining Treatment (POLST) form, which is another document used to specify a person’s wishes for medical treatment, including CPR.  

The classes will take place the first Thursday of every month, starting Aug. 2, from 2 to 3:30 p.m. in conference rooms 1 and 2 in the back of the hospital.

Drop-ins are welcome but registration is requested, either by registering online or calling Sedlacek at 541-451-7129.
