Samaritan Lebanon Community Hospital offers a monthly class for anyone with questions about how to fill out an Advance Directive form.

The Advance Directive is a set of instructions that expresses a person’s health care wishes if they were ever in the hospital and unable to speak for themselves. The Advance Directive tells the medical team who can speak for the patient and what the patient’s medical wishes are. While this document can feel overwhelming, these classes will attempt to make the confusing feel more understandable.

During each session, Chaplain Wes Sedlacek, will provide education and will give people the opportunity to complete their own Advance Directive. Other topics will also be mentioned, such as “quality of life” and the Portable Orders for Life Sustaining Treatment (POLST) form, which is another document used to specify a person’s wishes, especially about CPR.

The classes take place the first Thursday of every month (except January, July and December), from 2 to 3:30 p.m. in conference rooms 1 and 2 in the back of the hospital (behind the Garden Grounds coffee stand).

Drop-ins are welcome but registration is requested, either by registering online or calling Sedlacek at 541-451-7129.
