An Advance Directive is a legal document that allows you to communicate what type of life sustaining medical care you would want if you were in the hospital and ever became unable to speak for yourself. In other words, an Advance Care Directive is a set of instructions that express your health care desires if you are in a close to death medical condition and become incapable of letting others know what level of life sustaining interventions you desire.
